Your Bridge Between Hospital and Home

When you are hospitalized, all you can think of is getting better and going home. But there's no guarantee that when it's time for discharge, you'll be ready to function on your own. You might need ongoing, intensive physical therapy; speech therapy; wound care; or other forms of care you can't get at home. Maybe you live alone, and it's not safe for you to walk around or take a shower by yourself. You might need post-acute care to get back on your feet.

What is Extended-Care Rehab?

Post-acute care, also known as "skilled nursing" or "swing bed" care, is the continued care that happens between staying in the hospital (acute care) and getting back to your life at home. Arbor Health calls this Extended-Care Rehab. It is your bridge between hospital and home.

Nursing homes are another option for continued care. At Arbor Health, we contend that we are the best choice for many reasons. Just a few include:

  • We have a doctor on-site 24 hours a day.
  • We schedule for four to five patients per nurse. Nursing homes staff for an average of 20 patients per nurse.
  • We have on-site physical therapy, occupational therapy, speech therapy, respiratory therapy and certified wound-care staff—and more.
