Bridging Care Between Hospital and Home

It's quite common for patients to be well enough to be discharged from acute care hospitals, yet not well enough to care for themselves at home. This situation happens with head injuries, stroke, complex illness recovery, joint replacements, complex trauma and more.

Until Critical Access Hospitals like Arbor Health–Morton Hospital positioned themselves to specialize in this service line, patients discharged from acute care hospitals were transferred to nursing homes, where staffing levels run 10 to 15 patients per nurse, compared to four to five per nurse at Morton Hospital.
