Diabetes wellness services

Arbor Health diabetes wellness services are here to help prevent and manage diabetes through education, training and support. We offer custom designed individual and group sessions and twice a month support groups. Whether you are at risk or diagnosed, with pre-diabetes, type 1 or type 2 diabetes, we can help you gain important self-management skills.

Pre-diabetes and diabetes self-management support group

Our free pre-diabetes and diabetes self-management support group meets on the first Tuesday and also on the third Tuesday of each month at 10 AM in hospital conference room #2.

Prevent T2

Prevent T2 is a proven program to prevent or delay type 2 diabetes. The Prevent T2 Lifestyle Change Program is a year-long evidence-based program designed by the Center for Disease Control for people at risk for or diagnosed with pre-diabetes and their support person.

Risk factors include:

  • Being overweight.
  • Inactive lifestyle.
  • Over age 45.
  • History of high blood pressure.
  • Family member with diabetes.

These proven, evidence-based sessions are weekly for six months and then monthly for six months. Participants work with a lifestyle coach who helps you set goals, track your eating and activity, assess your progress, and solve problems. Call 360.496.3591 now to arrange for your first session!

Type 2 diabetes custom designed self-management education and support

These services are for those newly diagnosed or those in need of refresher sessions. It is for:

  • Those who have recently received a diagnosis of type 2 diabetes or pre-diabetes.
  • Those previously diagnosed who have had little or no diabetes education or who need a refresher class.
