Diabetic Educator (Part-time)
This Registered Dietitian Nutritionist (RDN), Certified Diabetes Educator (CDE) will work in partnership with clinical team members in primary and specialty care, to optimize patient health by providing individual and group diabetic self-management (DSME) and medical nutritional therapy (MNT) services for patients with chronic disease. This individual will serve as a member of a collaborative multidisciplinary healthcare team to assist as a liaison in diabetes population management and enhance the overall well-being of those suffering from chronic diseases that can be impacted by better nutritional lifestyle management.
- Bachelor’s degree in nutritional sciences (Master’s degree in Nutrition, Health or related field preferred)
Registered Dietitian Nutritionist (RDN)
Washington State Department of Health Dietitian Certification (CD)
Certified Diabetes Educator (CDE) or Board Certified-Advanced Diabetes Management (BC-ADM) completed or within 6 months of employment
1 years’ experience in clinical outpatient practice in a primary care and/or diabetes/endocrinology setting (3 years preferred).
Insulin pump and continuous glucose monitoring patient training experience
1+ year practice experience with diabetes patient medication management training and adjustment per protocols
Primary job responsibilities:
- Demonstrate competency in use of: insulin syringe, insulin and injectable pen devices, glucagon pens, insulin pump and continuous glucose monitoring systems, blood glucose meters and downloading data software and web-based programs for aforementioned devices.
- Ability to assess patient preferences and beliefs; health literacy and numeracy; and psychosocial factors, including food insecurity, housing stability, financial barriers, mental illness and chemical dependency, and apply that information to optimize patient health outcomes and health-related quality of life.
- Strong customer/patient focus.
- Demonstrate competency in basic mathematics for nutrition prescription and safe medication dose calculations.
- Ability to provide technical advice, guidance, and support to colleagues and other clinical staff in area of specialty.
- Excellent/advanced verbal and written communication skills in English via in-person, phone and electronic messaging.
- Proficiency in the use of an electronic medical record (EMR) system (i.e. Centricity, Epic) and basic computer applications (Microsoft Office, Outlook).
- Ability to multi-task and work with ambiguity.
- Complies with HIPAA/Confidentiality and workplace safety policies and procedures.
- Ability to remain calm and professional when faced with difficult situations or emergencies.
- Strives to demonstrate predictable, reliable, and timely attendance.
