Certified Nursing Assistant
The Certified Nursing Assistant is responsible for performing and documenting, as appropriate, specific patient/resident care delivery at the direction and under the supervision of an RN.
- High school diploma or equivalent required.
- Current Nursing Assistant Certification in the State of Washington with annual renewal required. Current American Heart Association BLS Certification or must be obtained within 30 days of hire.
One year recent experience as a nursing assistant in a patient/resident care setting preferred.
Primary job responsibilities:
A. Assists RN in physical care of patients/residents.
- Delivers patient/resident care in a safe and expedient manner.
- Provides bathing, oral care, AM and bedtime care as directed and meets other hygiene needs.
- Changes linens as appropriate.
- Sets patients/residents up for meals and assists with feeding as directed.
- Utilizes correct body mechanics when transferring/lifting.
- Turns, transfers, ambulates, and positions patients/residents as directed.
- Demonstrates skill in maintenance of range of motion.
- Assists patients/residents with elimination needs.
- Records I and O accurately and reports abnormalities to the appropriate team member.
- Provides water and ice routinely.
- Takes vital signs as directed. Records and reports to the appropriate person in a timely manner.
- Weighs patients/residents and measures height as directed and correctly documents.
- Answers call lights promptly and responds to patient/resident requests.
- Immediately notifies RN of change in patient/resident condition or other problems.
- Assists in performing after-death care as appropriate.
B. Accepts responsibility for the physical appearance of the facility and assures equipment availability.
- Picks up clutter and keeps equipment stored neatly and out of patient and resident hallways.
- Keeps facility equipment, such as wheelchairs, commodes, etc. readily accessible when needed.
- Reports equipment repair appropriately.
- Keeps rooms neat and uncluttered.
C. Participates in holistic patient/resident care. Provides objective and accurate information to the RN through documentation on appropriate forms.
- Demonstrates sensitivity to patients'/residents’ emotional, social, and mental health needs.
- Participates in plan of care and nursing reporting process.
- Identifies the psycho-social characteristics of patients/residents.
- Identifies adaptations necessary to accommodate age-related needs.
- Approaches age-related needs appropriately
- Pediatric through adolescent patients/residents.
- Geriatric patients/residents.
- Provides training for self care according to patient/resident capabilities.
- Assures patient/resident compliance with safety precautions.
- Demonstrates knowledge of hospital emergency procedures and codes.
- Provides adequate protection of patient/resident during care and transport.
D. Contributes to an efficient, cohesive, and coordinated facility by using good communication skills.
- Answers call lights promptly and delivers messages to appropriate staff members.
- Conducts patient/resident care in a professional manner while demonstrating a caring and compassionate attitude.
- Documents care as directed.
- Accepts direction and guidance from RNs
- Responds to stressful situations in mature, calm manner.
E. Adheres to nursing standards and regulations. Seeks continuing education for professional knowledge and contributes to the development and growth of the department.
- Completes annual mandatory professional education requirements.
- Attends a minimum of 16 hours of continuing education in health care/nursing per year.
F. Performs other related duties as assigned.
- Follows guidelines established at time of assignment by RN.
- Floats to other areas as instructed by RN or Supervisor.
- Accepts assignments for rapid response by the Charge Nurse.
- Works as transporter when needed.
- Accepts assignments willingly.
- Prioritizes workload to ensure timely completion of assignment.
- Asks appropriate clarifying questions relative to scope of assignment.
The primary duty of a Restorative Aide is to ensure maintenance of a patients optimum level of function. The patients on this program are encouraged or assisted to achieve and maintain their highest level of self-care and independence. These services are performed daily. Individual resident goals are established with input from nursing, therapies and aides and are evaluated at least quarterly.
- Observes and reports physical and/or changes in behaviors to Custodial RN.
- Completes all EHR documentation as appropriate.
- Properly uses and cares for all equipment and supplies used in the restorative program.
- Maintains hospital safety equipment for fall prevention, matching alarms with equipment, replacing batteries, etc. Makes requests to purchase equipment as needed to the Custodial Coordinator.
- Works cooperatively and productively with coworkers. Assists in the dining room for breakfast and lunch, passing trays, feeding residents, cleaning up etc.
- Responds rapidly and appropriately to emergencies involving residents/patients and coworkers safety.
- Follows the Nursing Department's policies and procedures for patient/resident care.
- Attends required staff meetings, attends any restorative meetings as scheduled.
- Projects a positive image of the organization in dealing with all resident/patients and public.
- Provides basic restorative programs as developed for residents by OT, not going outside of the plan without permission.
- Works with residents in compatible groups or individually as needed.
- Relate restorative activities with regular daily activities whenever possible.
- Work as an NAC as scheduled or requested by supervisor.
- Acts as a back up for the Activity Director for an afternoon activity, as needed.
- Other duties as assigned.
