Description:

Provides complex and diversified support to the Care Team (provider, medical assistant, nurse, etc.) by coordinating care for patients. Focus on health maintenance and those with serious, complex, or chronic health problems, or those with psychosocial issues. Work from a team approach, and in continuous partnership with patients and providers to promote: timely access to needed care, comprehension and continuity of care, and the enhancement of patient well-being.

Minimum qualifications:

  • Bachelor of Arts, Associates in Arts, Medical Assistant or LPN.
  • Preferred: Master’s degree in Social Work or Psychology. (Preference given if your degree program or professional training emphasizes primary care behavioral health coursework or clinical practicum experience)
  • Current Medical Assistant, Licensed Practical Nurse license if appropriate. Current Healthcare provider CPR certification, American Heart Association BLS, or obtain within first 6 months of employment.
  • Preferred: License for independent clinical practice, or seeking licensure, in the State of Washington or have the necessary clinical training to acquire licensure. Any one of the following licenses is preferred for this position:
    • Licensed Independent Clinical Social Worker (LICSW/A)
    • Licensed Mental Health Counselor (LMHC/A)
    • Licensed Marriage and Family Therapist (LMFT/A)
    • Licensed Advanced Social Worker (LASW/A)
  • Active State of Washington RN
  • At least one year relevant experience in a clinical setting required, outpatient care and service of patients with chronic disease preferred.
  • Knowledge of medical terminology.
  • Philosophy and values consistent with a patient-centered care model.
  • Professional and effective written and verbal communication skills with the ability to identify and employ communication strategies appropriate to the audience.

Primary job responsibilities:

  • Collects data and contributes to goal-oriented care and treatment plans, such as patient records, interviews, observation and team members. Coordinates team-based care through effective partnerships with patients, their caregivers, and their providers.
  • Work is generally performed independently requiring judgment and problem-solving skills with limited supervision to proactively identify patients in the practice for specific diagnosis and preventative care needs.
  • Develop and maintain patient registry to evaluate and analyze patient records based on Primary care “best practices” for patient who need assistance navigating through the healthcare system. May facilitate access to specialty providers as well as other care coordination team support specialists (e.g. Diabetes Educator, Registered Dieticians).
  • Serves as the point-of-contact, advocate and informational resource for assigned patients, family, care team and community resources. Proactively acts as patient advocate; responding with empathy and respect to resolve patient/family concerns.
  • Serve as a medical home quality improvement team member, assist with improvement of work flows through PDSA Cycles and measurement of quality indicators. Participates in care coordination, discharge planning and patient navigation meetings and training opportunities.
  • Participate in team huddles and evaluation of team data for proactive panel management. Reviewing clinic schedules and patient charts to assist the care team in coordinating care for visits and identifying preventive healthcare needs. Scheduling patients for preventative care needs after communicating with patient/family.
  • Work with clinic team on developing standards, implementation, and maintenance of programs in chronic disease management for patients. Maintains awareness of policies and procedures and performance standards. Actively develops and maintains professional and clinical skills.
  • Additional clerical duties as assigned.
